Administering of Medicines
If you would like the office staff to administer medicine during the school day, you need to complete and return a medicine consent form.
The medicine requires a pharmacy label with your child’s name and the following information:
- Pupil’s Name
- Name of Medicine
- Dose
- Frequency of Administration
- Date of Dispensing
Please download, sign and return your completed form to the school office in advance.
